2025-26 New Advisor Application Complete Packet - (Due by 2/15/26)

BCI Form/Reimbursement - (For Returning Advisors who need BCI checks completed. Due by February 15th.)

Club Annual Report - (Filed annually to retain the clubs charter, exempt status, etc. Due by January 15th)

Ohio 4-H Project and Resource Book Order Form - (Used to order club books for the year. We will only be keeping a limited stock of project books on hand, orders will be placed weekly as they are received in the OSU Extension Office.)

Honor Club Score Sheet - (Due by September 1st - Used to track points for Honor Club status.)

Club Attendance Sheet- (Used to track member attendance. Due by January 15th)

Financial Summary Sheet - (Recap of the club treasury for the 4-H year. Due by January 15th)

Ohio 4-H Member Transportation Waiver Form - (Used to grant permission for 4-H member to be transported by someone other than parent.)

Occasional Quantity Cooks Online Course Directions - this course is required whenever OSU Extension Volunteers are involved in the preparation and/or service of food. Read the policy here.
